Sex: ALLMin age: 19 Years

Inclusion Criteria: * 19-100 years of age. * patient presenting to the UAB Highlands Pain Treatment Clinic for initial evaluation and treatment or subsequent follow-up evaluation and treatment. Exclusion Criteria: * \<19 or \>100 years of age * a current prisoner
