Investigators will evaluate the effectiveness of two different EHR-embedded nudges to improve referral to cardiac rehabilitation.

Two types of nudges: 1) an auto pended bot order for cardiac rehab referral, 2) an EHR-embedded alert prompting cardiac rehab referral

Eligibility

Sex: ALLMin age: 18 Years

Medical Language ↔ Plain English

Inclusion Criteria: * Age of at least 18 years * History of Coronary Artery Bypass Grafting (CABG), Percutaneous Coronary Intervention (PCI), or valve repair/replacement in the past year Exclusion Criteria: * Hospice * Active home health

Primary Outcomes

Percentage of patients eligible for cardiac rehab referral who are referred for cardiac rehab

Time frame: Baseline

Secondary Outcomes

Percentage of patients who attend cardiac rehab
