Effects of Virtual Reality-Based Education on Pre-treatment Anxiety, Post-treatment Experience, and Satisfaction Among Adult Patients Diagnosed With Cancer Receiving External Beam Radiotherapy in Oman

Introduction: External Beam Radiotherapy is a common strategy used in the treatment of various cancers, and the patient undergoing treatment using this strategy many times experiences anxiety and stress. While patient education is essential for anxiety management, traditional methods may not adequately address the complex needs of patients in Oman, where no standardized pre-treatment education protocols exist. Virtual Reality (VR) technology offers promising potential for enhancing patient education and reducing anxiety. Purpose/Aim: This study aims to evaluate the effectiveness of VR-based education in reducing anxiety levels, improving treatment experience, and increasing satisfaction among adult patients with cancer receiving external beam radiotherapy (EBRT) in Oman. Research Design: A randomized control trial will be conducted with 122 patients undergoing external beam radiotherapy from SQCCCRC (Sultan Qaboos Comprehensive Cancer Care \& Research Center) and the National Oncology Center (NOC) in Royal Hospital. These patients will be randomly allocated to either the intervention group (VR-based education) or the control group (standard education). The intervention group will view an educational VR video via immersive headsets, while the control group will receive standard care. Study Instruments: Anxiety levels will be measured using the State Anxiety Inventory (SAI), the Radiotherapy Experience Questionnaire (RTEQ) will be used to measure treatment experience, and the Patient Satisfaction Questionnaire (PSQ) will be used to measure patient satisfaction. Plan for Data Analysis: The Statistical Package for Social Sciences (SPSS) version 29 will be used for data analysis. Descriptive statistics in terms of mean, standard deviation, frequencies, and percentages will be used to summarize patient characteristics. Paired t-tests will be used to compare pre- and post-intervention anxiety levels within each group. Independent t-tests will be used to assess differences between the intervention and control groups. Analysis of Covariance (ANCOVA) will be used to control confounding variables such as age, education level, and baseline anxiety scores. Expected Outcomes: The researcher predicts that VR education will lower anxiety levels while delivering better patient experiences and higher satisfaction than traditional education methods. The abstract nature of radiation and its potential side effects can be difficult to visualize, leading to heightened anxiety and misperceptions. In recent years, virtual reality (VR) has emerged as a promising tool for enhancing patient education and support in various healthcare settings. VR offers an immersive, computer-generated simulation of real or imagined environments, providing users with a sense of presence and interaction. In the context of cancer care, VR can illustrate the objectives and processes of radiotherapy in a visually compelling manner, which is difficult to achieve with traditional methods, by allowing patients to virtually experience the radiotherapy process. VR interventions have been associated with significant reductions in anxiety. Cancer Care in Oman: Cancer is a significant health concern in Oman and is the leading cause of death, accounting for 6% of all disease-related mortality. The Oman National Cancer Registry, established in 1985, plays a crucial role in monitoring cancer incidence and trends; as a result, cancer diagnosis notification has been mandatory since 2001, ensuring comprehensive data collection. Radiation therapy was not available in Oman; the government sponsored approximately 250 patients annually to receive radiation treatment abroad before 2004. The National Oncology Center (NOC) at the Royal Hospital in Muscat, launched in 2004, is the oldest center and treats around 70% of all cancer cases in Oman. External Beam Radiotherapy External Beam Radiation Therapy (EBRT) is an external irradiation source, usually a linear generator, that is used in this procedure to precisely administer high levels of radiation to cancer regions while lowering the dosage to nearby healthy tissues. To meet patients' psychological and physical requirements as they progress throughout the complicated radiation treatment (RT) process, patient education is crucial. Patients may experience physical and psychological impacts from RT and cancer treatment. Many patients experience anxiety, particularly at the commencement of treatment. Traditional methods include slides, printed materials, and in-person interactions, as well as video materials that have improved patient satisfaction and decreased anxiety. Virtual Reality (VR) is a powerful tool for patient education, as immersive virtual experiences can decrease anxiety. Virtual integration of patient education in radiotherapy (VIPER), using videoconferencing, is a feasible and acceptable method for patient education. Although the combined client-provider decision-making guidelines allow the multidisciplinary healthcare team to repeat and strengthen training and give patients several chances to raise concerns, it can also result in unequal education, misunderstandings, and false presumptions. One typical emotional reaction to malignancy is anxiety while preparing for radiotherapy. Long before the initial consultation, patients want to know about the radiation process. A patient's request for details is influenced by several aspects, such as who they are interested in receiving it from, at what time they want it, and in what style. The Radiotherapy Experience Questionnaire (RTEQ) can measure how patients experience the external RT session process and the environment in the treatment room. Virtual Reality in Healthcare: Over the past two decades, VR has found various applications in healthcare, offering immersive and non-immersive experiences. VR has evolved through two generations of technology: First-generation SLEs used machines to replicate real-life experiences, while more recent advancements include sophisticated software and hardware. VR enhances patient understanding of medical treatments like radiation therapy. For instance, VR can be used to educate patients about the radiotherapy process, reducing anxiety. VR serves as a distraction intervention to relieve symptoms such as anxiety. It is used during medical procedures to reduce anxiety. Studies have shown VR's effectiveness in reducing anxiety, particularly in patients undergoing medical interventions and cancer treatment. VR offers oncology sufferers emotional, physical, and emotional assistance. Virtual Reality and Patient Education: Virtual Reality (VR) can be enjoyable, which is important for learning, as a pleasant experience means higher engagement and understanding. VR educational sessions reduce their anxiety levels and allow patients to utilize audio and geographical aspects during radiation treatments to have an actual treatment experience. Personalized VR experiences can give patients a better understanding of how radiotherapy will be used to treat their cancer and can decrease anxiety. Cancer-related anxiety can lead to impaired social functioning and fatigue. Anxiety may arise from various reasons, including reaction to the cancer diagnosis, severe pain, long-term treatments, treatment side effects, and feeling of burden or dependence on others. Procedural anxiety can occur during or in anticipation of the procedure and may lead to behavioral disruptions, such as avoiding or terminating medical procedures. High levels of anxiety can negatively impact health, increasing treatment nonadherence, promoting unhealthy lifestyle factors, and chronically activating stress hormones. Anxiety can affect patients' learning capacity. Anxiety levels decrease once RT starts, falling from 29.9% to 17.4%. The State Anxiety Inventory (SAI) is commonly used to measure procedural anxiety. To minimize anxiety, successful instruction needs to tackle patient issues, give sufficient details for consent to be informed, and get patients ready for the RT procedure, addressing potential adverse outcomes. Non-pharmacological interventions such as virtual reality (VR) have shown promise in reducing anxiety. Relaxation techniques and counseling are also recommended. The Radiotherapy Experience Questionnaire (RTEQ) evaluates the patient's experiences of the external RT method, including anxiety and adapting throughout the external radiation therapy technique, as well as aims to evaluate the patient's impressions of the RT procedure and the therapeutic area setting.
