This prospective observational study aims to evaluate the association between intraoperative perfusion variability and postoperative troponin elevation in geriatric patients undergoing major oncologic surgery. Intraoperative hemodynamic and perfusion-related parameters will be recorded, and postoperative troponin levels will be assessed to investigate whether intraoperative perfusion changes are associated with myocardial injury. Troponin measurements will be obtained preoperatively and at 24 hours after surgery

Change in Serum Troponin Concentration From Preoperative Baseline to 24 Hours After Surgery
Postoperative troponin elevation will be assessed by comparing serum troponin levels measured preoperatively and 24 hours after surgery. Its association with intraoperative perfusion variability will be evaluated.
Time frame: Preoperatively and 24 hours after surgery
Association Between Intraoperative Perfusion Index and Postoperative Troponin Elevation
Perfusion Index (PI) will be continuously monitored intraoperatively and recorded at 15-minute intervals. The association between intraoperative PI values and postoperative troponin elevation at 24 hours after surgery will be evaluated.
Time frame: Intraoperative monitoring from the start to the end of surgery; troponin measured preoperatively and at 24 hours postoperatively
Association Between Intraoperative Pleth Variability Index and Postoperative Troponin Elevation
Pleth Variability Index (PVI) will be continuously monitored intraoperatively and recorded at 15-minute intervals. The association between intraoperative PVI values and postoperative troponin elevation at 24 hours after surgery will be evaluated.
Time frame: Intraoperative monitoring from the start to the end of surgery; troponin measured preoperatively and at 24 hours postoperatively.
Association Between Intraoperative Cerebral Oxygen Saturation and Postoperative Troponin Elevation
Cerebral oxygen saturation measured by near-infrared spectroscopy (NIRS) will be continuously monitored intraoperatively and recorded at 15-minute intervals. The association between intraoperative cerebral oxygen saturation values and postoperative troponin elevation at 24 hours after surgery will be evaluated.
Time frame: Intraoperative monitoring from the start to the end of surgery; troponin measured preoperatively and at 24 hours postoperatively.
